Please fill out this entire form and be sure to mark whether the player accepts or declines the team placement offered. If the player accepts, your commitment check will be cashed and the coaches will be in touch later. If the player declines, the commitment fee check will be destroyed.
We have included the Player/Parent contract with this form rather than doing it on paper. You are required to e-sign that as well, in order to accept a position and instructions are on the next page.
